68yo female patient sustained a fall on an outstretched hand. Plain x-rays revealed a closed fracture to the right distal radius and ulna. This was reduced by the orthopaedic surgeon and held in a forearm plaster of Paris. Post op x-rays showed satisfactory alignment and she was discharged to see her GP with follow up with the orthopaedic team at a later date.

Being a good GP, in addition to checking that her neurovascular bundle is intact, you noticed that she was unable to extend her right thumb. You organised for an urgent soft tissue ultrasound.

Ultrasound revealed a rupture of the extensor pollicis longus tendon of compartment 3 at the wrist with the proximal stump at 32mm from the radial head and the distal stump 6mm from the radial head.

She was referred to the hand surgeon.
