Criteria
- Active foot ulcer or infection.
- History of a foot ulcer, amputation, or infection (below the ankle).
- Suspected or diagnosed Charcot’s neuro-arthropathy.
- Hospital admission for a foot complication in the previous 12 months.
- High risk foot due to two or more risk factors, including:
- peripheral neuropathy.
- major foot deformity e.g., claw toes or bunions.
- significant peripheral arterial disease.
- Suspected osteomyelitis or soft tissue infection of the foot.

- Increased risk of lower extremity foot ulcer and/or amputation due to one or more of the following risk factors:
- Peripheral neuropathy
- Major foot deformity e.g., claw toes or bunions
- Peripheral arterial disease
- Secondary prevention services for people with a history of ulceration/amputation.
- Foot assessment (including education) for patients with diabetes
- History of foot ulceration, lower extremity amputation, and/or charcot arthropathy

- Preventative care for people with moderate to severe PVD.
- Foot assessment and services for people on haemodialysis and foot related risk factors.
