  • Active foot ulcer or infection.
  • History of a foot ulcer, amputation, or infection (below the ankle).
  • Suspected or diagnosed Charcot’s neuro-arthropathy.
  • Hospital admission for a foot complication in the previous 12 months.
  • High risk foot due to two or more risk factors, including:
    • peripheral neuropathy.
    • major foot deformity e.g., claw toes or bunions.
    • significant peripheral arterial disease.
  • Suspected osteomyelitis or soft tissue infection of the foot.

  • Foot assessment (including education) for patients with diabetes
  • History of foot ulceration, lower extremity amputation, and/or charcot arthropathy
